  • Type: Preferred Supplier Organization (PPO)
  • Preventive services: 100% coverage when networked; includes two routine oral exams and cleanings per year and one set of bitewing x-rays per year
  • Basic services: 60% coverage when networked; includes emergency care for pain relief and two fillings per year
  • Supplements: $100 per year for in-office teeth whitening
  • Annual deductible: $50 individual deductible and $150 family deductible

  • Basic services: Paid according to an established fee schedule. Includes fillings, extractions and x-rays
  • Annual deductible: None
  • Available states: The Dental Value HI215 plan is available in Florida, Missouri, Ohio, Tennessee and Texas. Another option, the Dental Value C550 plan, is offered in Georgia, Illinois and Kentucky

Preventive Plus dental plan

  • Type: PPO
  • Annual maximum: $1,000 per person
  • Preventive services: 100% covered; includes exams and cleanings
  • Basic services: 50% covered; includes fillings, extractions and x-rays
  • Annual deductible: Described as “weak” (but not specified)

  • Basic services: 80% covered after deductible. Includes extractions and root removal, among other services
